That's true, moderation is usually the way to go for this.
The only way to surround this is to use the serendipity_event_adduser plugin and enable comment-posting only for registered users. If that is activated, only logged in users can comment, and then the comment username is always set to the user comment name, so other people cannot fake their identity.
This plugin of course disables anonymous comment posting, which might not be what you want. The plugin could be enhanced to check a username against the userdatabase; if such a username is found, it can only be used if the user is logged in, and the comment is rejected. Actually this could be implemented into the spamblock plugin as well...
But I must admit I don't think this is worth the effort; usually if you pay attention to your blog, you would moderate such things.
