Theme demos on blog.s9y.org don't work
I'm not sure if this has been intentionally abandoned or not. However, there are still links on the Themes page to view a demonstration of the theme on the official s9y blog, which will show with its default theme regardless.

Together with your "Sometimes it works, other times, nothing.", I'm pretty sure this is not a Serendipity issue. See the "Not a JPEG file"? That could explain why it sometimes works – apparently the GD library (which PHP uses in your case to process images) can not process this ...
