I think your database is unhappy. Is this your server or a shared hosting account? If it's someone else's job to maintain the db server you need to tell them to fix it.
If it's up to you I think you need to make sure mysql's user has permission to write to /tmp and restart mysql.

I assumed it was a browser bug because it happens in Firefox and Konqueror but not in Opera-- the preview iframe works fine as is using Opera.
