I'm playing with the contact form plugin. I'm using a slightly modified standard style, but the layout of the contact form is broken.

Any hints how to fix this??

It's highly likely that there is a closing tag missing somewhere in here. The "usual suspect" would be a missing </div> causing the layout to fail. Since it's working on other pages of the blog, it's probably in /templates/YOUR_TEMPLATE/plugin_contactform.tpl – unless you don't have a modified version of that file in your template.
Where does the checkbox related to saving the IP come from, did you add that manually or is it added by some plugin?
